New England Center and Home for Veterans is seeking a Safe Haven Day Case Manager to provide clinical case management and supportive counseling for Veterans in a 25-bed, 24/7 Safe Haven program. The role emphasizes Motivational Interviewing, Harm Reduction, Relapse Prevention, and Trauma-Informed Care, with essential staff duties including safety, service coordination, and discharge planning.

Qualifications

• Bachelor's degree in a human services related field or equivalent experience.
• 3-5 years' experience in case management with substance use disorders and mental illness.
• LADC/CADC certification a plus.
• Knowledge of housing first, harm reduction, motivational interviewing, relapse prevention and trauma-informed care.
• Crisis intervention and de-escalation skills.
• Strong communication and interpersonal abilities.

Responsibilities

• Collaborate with program staff and VA Liaison within a 25-bed, 24/7 program.
• Manage caseload of Veterans not yet engaged in abstinence or treatment; connect to services.
• Provide individual supportive counseling using evidence-based methods.
• Maintain safety, manage crises, and coordinate with Operations Staff.
• Assist Veterans toward permanent housing, increased income/benefits, and self-determination.
• Complete biopsychosocial assessments and discharge plans with each Veteran.
• Coordinate with Human Services and Operations staff to provide coordinated services.
• Ensure data collection and documentation deadlines are met.
• Provide community referrals and appointment accompaniment during discharge planning.
• Facilitate rehabilitative and social skills activities.
• Attend department meetings, case conferences, and review boards.
• Perform other duties as assigned.
