You will coordinate service plans, assist with housing goals, and connect Veterans to VA and community resources, while working closely with VA personnel and the care team to ensure holistic support and progress tracking.

Qualifications

• Military/Veteran experience or strong familiarity with military culture.
• Bachelor’s degree in Social Work or a related field preferred; High School Diploma/GED required.
• Two years of related experience preferred; Military/Veteran experience strongly preferred.
• Knowledge of Veteran services and community resources is a plus.
• Valid First-Aid certification required; Must pass a criminal background check.

Responsabilités

• Provide case management and residential support to Veterans.
• Assist residents with service plans, housing goals, and independent living skills.
• Connect Veterans with VA, medical, social, educational, and community resources.
• Assist with housing searches, appointments, outreach, and discharge planning.
• Facilitate structured groups and living skills activities.
• Monitor residents’ progress and well-being; maintain accurate documentation and progress notes.
• Collaborate with VA personnel, families, providers, and the care team.
• Assist residents with daily living activities when needed.
