Qualifications

• 1 year of experience in case management.
• Preferred degree in Psychology, Social Work, Rehabilitation Counseling or related field.

Responsibilities

• Manage a caseload of homeless veterans.
• Link veterans with VA and community services.
• Conduct intake assessments and case documentation.
• Develop and monitor individual service plans.
• Coordinate referrals to programs and facilities.
• Educate veterans on community resources.
