Qualifications

• Provide case management and supportive services to veterans toward housing, employment, education, financial stability, and wellness goals.
• Coordinate community resources, benefits assistance, healthcare referrals, housing support, and employment services.
• Create and maintain Individual Action Plans, monthly reviews, discharge summaries, and compliant case documentation.

Responsibilities

• Conduct comprehensive client assessments to identify needs, barriers, and goals.
• Develop and monitor Individual Action Plans and track progress.
• Coordinate services with VA, healthcare providers, and community organizations.
• Complete intake assessments, verify eligibility, and document interactions.
• Conduct home visits to monitor safety and housing stability.
• Facilitate life skills workshops focused on employment readiness and finances.
