Job summary

Tarzana Treatment Centers, Inc. is seeking a Case Manager I / Veteran Services Case Manager in Northridge, CA to support veterans at elevated risk for suicide through individualized case management and connection to VA, housing, behavioral health, and wellness resources.

The role involves outreach, required screenings, service plan development, benefits advocacy, and coordination of transportation and referrals with a focus on continuity of care and client stability.

Qualifications

• Bachelor’s degree in a related field (minimum).
• At least one year of case management or housing navigation experience; veteran-specific experience preferred.
• Valid driver’s license and insurance (preferred).

Responsibilities

• Conducts self in a professional manner and upholds agency policies.
• Coordinate outreach, screenings, and development of individualized service plans.
• Advocate for benefits and arrange referrals to VA, housing, behavioral health, and other services.
• Maintain progress notes documenting contacts and progress toward goals.
• Assist with employment/vocational referrals and re-engagement in treatment.
