Job summary

WestCare Foundation in Minnesota seeks a Case Manager to provide intensive, person-centered services to veterans and service members at risk of suicide. You will coordinate mental health care, housing, employment support, and wraparound resources to support stabilization and long-term wellness.

Responsibilities include comprehensive assessments, individualized service plans, crisis collaboration, and timely documentation, with travel within the service area and occasional evenings or weekends.

Qualifications

• Minimum of 2 years of experience in case management, behavioral health, or veteran services.
• Bachelor's degree in social work, psychology, human services, or related field preferred.
• High school diploma or equivalent required.

Responsibilities

• Conduct comprehensive assessments to identify strengths, needs, and risks.
• Develop individualized service plans focused on reducing suicide risk.
• Provide ongoing case management, coordinating mental health, housing, and other services.
• Support crisis intervention with intervention specialists and peer support staff.
• Conduct regular follow-up to monitor progress and adjust care plans.
• Serve as liaison between participants and service providers with warm handoffs.
• Maintain documentation in compliance with grant requirements and confidentiality.
• Participate in multidisciplinary case conferences, trainings, and meetings.
• Assist with tracking grant outcomes and required reports.
• Maintain knowledge of local resources, VA services, and prevention strategies.
