Job summary

Tarzana Treatment Centers, Inc. seeks a Regular Full time Case Manager to provide services to patients on an individual caseload in Tarzana, CA. The role reports to the Program Director and aligns with the agency’s treatment philosophy.

The position requires at least 1 year of behavioral healthcare experience, enrollment in Addiction Studies, and a valid driver’s license. Benefits include medical, vision, life insurance, and a 401(k), with opportunities for continuing education and career

Qualifications

• Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP.
• 1 year experience in a Behavioral Healthcare setting.
• Valid driver’s license and insurance.

Responsibilities

• Provides case management services to patients assigned to individual caseload.
• Inform patients about other Tarzana Treatment Centers, Inc. services that may help family members.
• Develops a comprehensive Individualized Treatment Plan with the care team.
• Facilitates discharge planning, housing, employment, medical/psychiatric needs, and aftercare.
• Maintains regular progress notes in the patient’s Medical Record documenting contacts and progress.
• Engages in outreach and follow-up to reengage patients in treatment and referrals.
