Job summary

WestCare Inc in Milwaukee, WI is seeking a full-time Case Manager to support veterans and families under the Sergeant Fox Suicide Prevention Grant. You will monitor progress, maintain confidential records, and advocate for participants while collaborating with clinical staff and community partners to ensure safety and well-being.

Qualifications

• Bachelor's degree required.
• Experience in case management or related social services preferred.
• Knowledge of veteran services and benefits systems preferred.
• Familiarity with trauma-informed care and suicide prevention principles preferred.
• Ability to obtain and maintain required background checks.

Responsibilities

• Conduct intake, eligibility verification, and comprehensive needs assessments.
• Develop participant-centered service plans promoting safety and wellness.
• Coordinate access to behavioral health, housing, employment, benefits, and related resources.
• Identify social determinants of health and barriers to care.
• Monitor engagement and progress toward service plan goals; adjust referrals as needed.
• Maintain regular contact through visits, calls, and community outreach.
• Collaborate with clinical staff and external providers for safety concerns.
• Escalate suicide risk per established procedures.
• Facilitate referrals and linkage to VA facilities and veteran service organizations.
• Maintain confidential records and progress notes; contribute to data collection.
