WestCare Tennessee is seeking a Case Manager to join the Prevention team serving veterans and their families under the SSPGF grant. You will provide case management, coordinate services, and link to community resources, employment help, and housing support.

The role requires strong communication, data tracking, and ability to work with veterans across counties; travel and driving license required; salary commensurate with experience.

Competenze

• Experience with veteran services and case management.
• Excellent communication, public speaking, and group facilitation.
• Strong organizational and data-tracking abilities.
• Proficiency with Microsoft 365 and basic data systems.
• Valid driver’s license and willingness to travel.

Mansioni

• Provide case management for eligible veterans and their families within the SSPGF program.
• Schedule eligibility appointments and review required documentation.
• Develop and review wellness plans and coordinate peer support and family interventions.
• Lead weekly case management meetings via virtual platform or phone.
• Assist with community resources, employment readiness, housing, and veteran services.
• Maintain case files and input data into VA tracking systems and WestCare databases.
• Collaborate with project staff to report outcomes and attend meetings.
• Participate in conferences, trainings, and promotional events as needed.
