Job summary

Endeavors is seeking a dedicated case manager to support Veterans and their families who are homeless or at risk of homelessness in Tyler, TX. The role focuses on assessment, service planning, and coordination of housing and community resources to improve quality of life.

Responsibilities include advocacy, intake, program monitoring, and collaboration with partners. Evening/weekend hours and travel within the county may be required, with a strong emphasis on compassionate, client-first service.

Qualifications

• Bachelor's degree in a relevant field.
• 1+ years of case management experience; 3+ years preferred.
• 2+ years in a customer service focused environment.
• Licences: LMSW, LBSW, LMFT preferred; valid driver’s license.

Responsibilities

• Advocate for Veterans' access to housing, utilities and community services.
• Conduct intake, assess needs, develop and monitor service plans.
• Coordinate with Financial Assistance Coordinator for payments to third parties.
• Meet with Lead Case Manager to review caseload and outcomes.
• Network with Veteran community groups and present program benefits.
• Perform other duties as assigned.
