Qualifications

• Bachelor’s degree in social science or related field; graduate degree/licensure preferred.
• Three years of experience working with low-income populations, preferably veterans/homeless.
• Strong communication skills (oral and written) and computer literacy.
• An approved writing sample demonstrating clinical documentation is required.

Responsibilities

• Provide outreach and case management for AWVH residents.
• Facilitate rapid transition (within 90 days) to permanent housing under SSVF.
• Collect data per NOFA funding requirements and ensure accurate HMIS reporting.
