Job summary

ACCESS is seeking a Housing Stabilization Case Manager II to work with Veterans and families to overcome barriers to housing stability. The role combines in-office and community-based work, including home visits, to provide wrap-around support and coordinate with a multidisciplinary team.

The position emphasizes confidential case management, data accuracy, and referral to internal ACCESS programs and external services to maintain housing stability for participants.

Responsibilities

• Maintain a caseload of high acuity Veteran households.
• Screen participants for eligibility for Federal and State assistance.
• Refer participants to ACCESS programs and external resources as needed.
• Maintain accurate case notes and complete data entry in systems.
• Coordinate with legal services to address housing-related legal barriers.
• Navigate community resources to support housing stability.
