The Salvation Army in Manhattan, KS is hiring a Case Manager for the Supportive Services for Veteran Families (SSVF) program. You will provide case management and rapid resolution, rehousing, and homelessness prevention services to veteran households, coordinating with VA and community partners to promote housing stability.

Responsibilities include outreach, intake and assessment, strengths-based planning, service coordination, and thorough documentation.

Qualificações

• BA/BS required; bachelor’s degree in social work preferred.
• 2–5 years of relevant experience preferred.
• Experience with motivational interviewing, mediation, and service delivery to high-need populations.

Responsabilidades

• Provide case management interventions to veteran households, including housing stability planning.
• Coordinate with VA, CoC, mainstream resources and other agencies.
• Conduct intake/assessment and required enrollment documentation.
• Perform outreach to veterans and community partnerships; participate in events.
• Document progress, referrals, and outcomes in HMIS and case files.
• Assist with staff training and on-boarding; maintain confidentiality and compliance.
