Job summary

Responsibilities

Execute a structured schedule of follow-up calls to Veterans at defined milestones during their first two years of housing.
Confirm current housing status and verify rent and utility obligations are being maintained.
Conduct wellness checks to identify needs such as food insecurity, mental health concerns, or employment challenges.
Serve as an early-warning system and coordinate timely intervention with the case management team.
Document every interaction and housing status in the CRM for long-term outcome tracking.
Provide warm hand-offs to VA programs or contracted providers for specialized services.
