Job summary

Tunnel To Towers Foundation seeks an experienced Case Manager to oversee services for Veterans transitioning from unsheltered homelessness to permanent housing. The role requires strong social work background, trauma-informed care, and collaboration with staff and community resources.

The ideal candidate will coordinate assessments, service plans, and ongoing support, ensuring data integrity and confidentiality while promoting stability and self-sufficiency for Veterans in a Veterans Village

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Master’s degree in Social Work or related field preferred.
• Minimum 3 years of case management experience in supportive housing or social services.
• Strong understanding of housing-first models and trauma-informed care.
• Experience using Electronic Health Record (EHR) systems.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and in a multidisciplinary team.
• Commitment to diversity, equity, and inclusion.
• Experience working with veterans or vulnerable populations.

Responsibilities

• Provide comprehensive case management services to Veterans, including assessments, service planning, and ongoing support.
• Develop individualized service plans based on Veterans' needs, goals, and strengths.
• Monitor and evaluate Veterans’ progress, adjusting service plans to ensure adequate support.
• Utilize EHR systems to document case notes, service plans, and Veteran interactions.
• Coordinate with community resources and support networks to meet Veterans' needs.
• Conduct regular visits to assess living conditions, wellness, and program compliance.
• Advocate for Veterans to ensure access to healthcare, employment, and benefits.
• Maintain accurate case files, with data integrity and confidentiality.
• Facilitate life skills workshops, employment readiness, and financial literacy sessions.
• Collaborate with multidisciplinary teams to support Veterans' stability and development.
• Provide crisis intervention and de-escalation support, connecting Veterans to emergency resources.
• Assist with housing retention strategies, eviction prevention, and conflict resolution.
• Ensure compliance with policies and trauma-informed practices.
