Job summary

The Tunnel to Towers Foundation seeks an experienced Social Services Case Manager for its Homeless Veteran Program in the United States. The role supports the Social Service Manager by managing a caseload of veterans and their families, navigating intake, and coordinating resources to address psychosocial needs.

Responsibilities include documenting communications, coordinating referrals, and maintaining organized files while ensuring compassionate, confidential service.

Qualifications

• Must have 3 years social worker experience.
• Master's degree in Social Work (CSWE-accredited) preferred (LMSW/LCSW).
• Knowledge of health care and social service system entitlements.
• Ability to communicate effectively in writing and verbally.
• Strong organizational and multi-tasking abilities.

Responsibilities

• Serve as the liaison among the Foundation, recipients, and families during intake and ongoing follow-up.
• Prepare all necessary documentation for potential recipients.
• Maintain current files and document all communications with recipients and families.
• Provide psychosocial advocacy and connect families with short-term or ongoing counseling resources.
• Ensure ongoing psychosocial needs are addressed through follow-up communication with families.
• Perform administrative tasks including email, phone correspondence, and research for resources.
