Job summary

U.S.VETS is seeking a Case Manager I in Los Angeles to guide veterans toward independence through collaborative case management. You will assess needs, plan services, and coordinate with VA and partners to ensure access to housing, employment, and health services.

Under supervision, you will document progress and support clients with action plans and life-skills development. Responsibilities include intake, file management, and monthly IAP reviews, plus home visits and data entry.

Qualifications

• High School Diploma required.
• One year of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Certification in Substance Abuse Counseling preferred.
• Experience facilitating support groups preferred.

Responsibilities

• Assess veterans' housing, employment, education, financial, and health needs.
• Coordinate care with VA personnel and community partners for eligibility and services.
• Facilitate intake and compile client files ensuring eligibility documentation.
• Develop Individual Action Plans (IAP) with supervision and sign-off.
• Maintain confidential client files and ensure HIPAA compliance per standards.
• Document progress notes in DAP format and update IAPs monthly.
• Conduct home visits to provide case management services and support health/welfare of clients.
• Assist in data collection, reporting, and HMIS data entry.
