Job summary

U.S.VETS is seeking a dedicated Case Manager II – Driver to guide veterans toward independence through comprehensive case management. You will assess needs across housing, employment, education, finances, and health, coordinating with VA and community partners for essential services.

You will also manage intake, develop Individual Action Plans, track progress, and ensure confidentiality and HIPAA compliance throughout all records.

Qualifications

• Assess veterans referred to the program to identify housing, employment, education or health needs.
• Coordinate care with VA personnel and community partners for eligibility and services.
• Facilitate client intake and assemble files to ensure program documentation is complete.
• Develop and monitor Individual Action Plans (IAP) with clients and track progress.

Responsibilities
