Job summary

U.S.VETS is seeking a Case Manager III to guide veterans toward independence. This exempt, on-site role focuses on assessing needs across housing, employment, education, and health, coordinating services, and supervising case management staff.

You will conduct intakes, develop Individual Action Plans, monitor progress, and ensure HIPAA-compliant documentation while collaborating with VA partners and program coordinators.

Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or related field is required.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Certification in Substance Abuse Counseling preferred.
• Experience facilitating support groups preferred.
• Strong oral and written communication skills.
• Professional manner and appearance.
• Ability to work independently and within a team.
• Intermediate computer literacy; must navigate EMR systems and Microsoft Office applications.
• Valid driver’s license; if driving company vehicles, must meet insurance requirements and complete training.

Responsibilities

• Assesses veterans referred to the program including housing, employment, education/training, financial, sobriety support, legal, family/social and medical/psychiatric needs.
• Coordinates care with VA personnel and community partners for eligibility documentation and health services.
• Facilitates client intake process and compiles client files.
• Collaborates with clients to develop Individual Action Plans (IAP).
• Provides guidance to staff on IAPs, intervention strategies, progress notes, and discharge planning.
• Conducts crisis intervention and supports staff with client issues.
• Maintains confidential client files per HIPAA and organization standards.
