Society of St. Vincent de Paul South Pinellas, Inc. in Lakeland seeks a Health Care Navigator to connect Veterans with VA/home care resources and coordinate care with medical teams.

This role emphasizes case management and collaboration with primary care providers to ensure timely access to services. The navigator acts as liaison between the SSVF and VA/community clinics, supporting Veterans with complex needs in navigating health care plans and services.

Qualifications

• Must be able to speak, write and understand English.
• Have basic knowledge of diversity and cultural sensitivity in client care.
• Ability to organize, prioritize, multitask, and meet deadlines.
• Excellent communication and listening skills.
• Evidence of strong work ethic and reliability.
• Willingness to undergo Level 2 background screening (Florida Clearinghouse).
• Valid Florida driver’s license if driving agency/personal vehicle.
• Reliable transportation.

Responsabilités

• Conduct assessments of Veterans with the interdisciplinary treatment team and family members.
• Assess Veterans’ strengths, limitations, risk factors, and supports to optimize access to care.
• Enter data in HMIS or equivalent system promptly and completely.
• Assist Veterans in communicating care preferences to support shared decision making.
• Serve as a resource for education and support for Veterans and families.
• Advocate for clients while integrating cultural values into care plans.
• Consult with team members to address Veterans’ needs.
• Identify barriers and work with leadership to find viable solutions.
• Coordinate services with other organizations to ensure comprehensive care.
• Participate in team meetings and case conferences to enhance care coordination.
