Job summary

Volunteers of America Colorado is seeking a Case Manager 1 to deliver community-based case management and care coordination for Veterans and families enrolled in the SSG Fox SPGP. This role involves outreach, needs-based assessments, and linking participants to VA and community resources.

The position supports trauma-informed practices, develops individualized service plans, and ensures progress toward stability and well-being.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• Colorado driver’s license and state-mandated automobile insurance.

Responsibilities

• Provide case management and care coordination for Veterans and families in the SSG Fox SPGP.
• Conduct needs-based assessments and coordinate services with VA and community providers.
• Develop individualized service plans addressing suicide risk, stability, and well-being.
• Document client interactions with required eligibility and demographic data.
• Engage in outreach to identify eligible Veterans and maintain program fidelity.
• Maintain confidentiality and collaborate across teams; participate in trainings.
