Job summary

WestCare Foundation in Fresno is seeking a Senior Case Manager to manage a caseload of low-income veteran participants, linking them with VA and community services under a Housing First model. You will lead housing stability efforts, coordinate plans, and advocate for clients to access needed resources.

You will collaborate with SJVV and community partners, conduct outreach, and ensure thorough documentation while supporting client empowerment and independence.

Qualifications

• Ability to work collaboratively in a professional, team-oriented environment.
• Ability to operate Microsoft Word, Excel and Outlook program formats.
• Veteran and/or military experience preferred.
• Must be able to obtain and maintain criminal background clearance and a valid driver’s license.

Responsibilities

• Serve as an active member of the San Joaquin Valley Veterans case management team and carry out responsibilities as needed.
• Support a larger caseload of homeless veterans and/or veterans at risk of homelessness.
• Create individual plans, monitor weekly and maintain exit plans.
• Coordinate referrals and placements to VA programs and community facilities.
• Educate and refer individuals to community agencies for food, clothing, employment, transportation, etc.
• Advocate for veterans to improve access to services and participate in outreach efforts.
• Maintain client files and documentation per policy and county guidelines.
