Job summary

Paycom - ATS seeks a Case Manager to provide outreach and case management for Homeless Veterans under the GPD VA grant. You will coordinate with the VA and community resources to support housing, employment and financial stability for residents.

The role requires developing treatment plans within 72 hours of admission, renewing plans every 90 days, and maintaining weekly progress notes to track outcomes. A Master’s degree in social work or related field is preferred, with a valid driver’s

Qualifications

Master’s Degree in Social Work, Human Services or Sciences (or combination of related Bachelor’s degree and three years related work experience).
Prior military experience preferred.
Knowledge of HUD/HMIS related to AWVH-G residents.
Valid Driver’s License; auto liability insurance if required.

Responsibilities

Provide community outreach and case management services to Homeless Veterans.
Maintain case management for all residents with focus on housing, employment and finances.
Develop initial treatment plan within 72 hours of admission and review every 90 days.
Maintain weekly notes summarizing residents’ status and progress.
Coordinate with VA weekly and with HUD-VASH case manager as needed.
Plan and facilitate group sessions; advocate for veterans’ rights.
