Job summary

Endeavors Inc. is seeking a dedicated Case Manager to support Veterans facing homelessness or at risk of homelessness.

You will advocate for access to housing, healthcare, and community resources, and coordinate referrals to services that promote housing stability and independence. You will conduct home visits, develop individualized service plans, document progress, and assist with essential documents while coordinating payments to providers and landlords.

Qualifications

• Bachelor’s Degree in Social Work, Sociology, Psychology or related field.
• 1+ years case management experience; 3+ years preferred.
• 2+ years in a customer service focused environment.
• LMSW, LBSW, LMFT preferred.
• Driver’s License with clear record.

Responsibilities

• Advocate for Veterans and facilitate access to housing assistance, utility assistance, healthcare, transportation, income support, and other resources.
• Develop and maintain a database of community resources and referral partners; coordinate referrals to services.
• Provide intensive, hands-on support and advocacy for Veterans experiencing chronic homelessness.
• Assist Veterans with completing housing and benefit applications; accompany to appointments as needed.
• Conduct home visits and follow-up contacts to monitor housing stability and progress toward service plan goals.
• Coordinate, monitor, and document service delivery and housing inspections; assist with obtaining identification and essential documents.
• Coordinate payments to landlords, utility providers, and vendors; maintain regular communication with Veterans.
