Job summary

Lakes-Regional-Community-Center is seeking a QMHP/Veteran Navigator to work independently providing veteran outreach, care coordination, benefits and resource navigation, and referral follow-up through the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program. The role helps participants identify needs and goals and connect with VA healthcare and community supports.

Qualifications

• Bachelor’s degree in human services required.
• Valid driver’s license and insurance required.
• Working computer skills including MS Office.
• Preferred: Master’s degree and bilingual English/Spanish.

Responsibilities

• Provide veteran outreach, care coordination, benefits and resource navigation, and referral follow-up through the grant program.
• Identify needs and goals; connect participants with VA healthcare and community supports.
• Complete assigned screenings after training.
• Maintain timely service documentation and collect required participant data.
• Collaborate with program staff, peer specialists, MVPN, Veteran Service Officers, and community partners; travel regionally for outreach events.
