Job summary

CenterPointe is seeking a Veterans Care Coordinator to help veterans and their families access services and suicide prevention resources. The role builds supportive relationships, identifies needs, coordinates personalized service plans, and connects individuals with VA services, health care, benefits, and community resources.

Qualifications

• High school diploma or equivalent.
• Two or more years of experience working in Behavioral Health.
• Knowledge of co-occurring disorders and related needs.
• At least 21 years of age.
• Valid driver's license with acceptable DMV record (last 7 years).

• Help veterans and families access services and support through our suicide prevention program.
