Job summary

Centerstone in Illinois is seeking a Veteran Care Navigator to provide in-person peer support to Veterans under the SSG Fox Suicide Prevention Grant. You will focus on suicide prevention, care coordination, and access to behavioral healthcare and services.

The role requires trauma-informed, person-centered approaches and collaboration with internal teams and community partners to promote engagement, continuity of care, and positive outcomes.

Qualifications

• High school diploma or GED required; Bachelor's degree is preferred.
• Military veteran status required.
• Must have one year experience in behavioral health field.

Responsibilities

• Provides Veteran peer support and suicide prevention-focused services.
• Conducts outreach, engagement, and follow-up with Veterans and eligible family members.
• Represents Centerstone to internal and external stakeholders and supports grant objectives.
• Provides community education and consultation related to suicide prevention and Veteran mental health.
• Maintains accurate, timely documentation in line with grant and regulatory requirements.
• Assists with program development, workflows, and quality improvement efforts.
