Job summary

Community Bridges, Inc. is seeking a Health Care Case Manager for its SSVF program in Phoenix. You will coordinate care for Veterans with complex needs, liaising with the VA and community clinics to ensure adherence to care plans and timely services.

You will work with an interdisciplinary team, provide health education, and advocate for patient-centered outcomes while maintaining sensitivity to diverse Veteran populations.

Qualifications

• Two years' experience in health care or social services.
• Experience with veterans or vulnerable populations preferred.
• Minimum one year of case management, resource coordination, and interdisciplinary teamwork.
• Familiarity with community resources preferred.
• Recovery from substance use and/or mental health disorders preferred.

Responsibilities

• Serve as liaison between SSVF Grantee and VA/community clinics for Veterans with complex needs.
• Provide case management, care coordination, health education, and administrative duties.
• Collaborate with primary care providers and multidisciplinary teams.
• Identify and address system challenges to improve care coordination.
