South Coast Business in Coos Bay, OR is seeking a Veteran-Directed Care Program Coordinator to coordinate, implement, and monitor the VDC program in collaboration with the VA, local healthcare partners, participants, and representatives.

The role focuses on guiding participants in directing their care, developing spending plans, coordinating caregivers, ensuring compliance with VA requirements, and maintaining accurate documentation while managing referrals and service delivery.

Formación

• Associate degree in human services or related field; relevant experience may be considered.
• Two+ years of program coordination, case management, or healthcare support experience.
• Ability to maintain confidential records and complete detailed documentation.
• Excellent organizational, communication, and problem-solving skills.
• Ability to work with Veterans, caregivers, and diverse communities.

Responsabilidades

• Coordinate day-to-day operations of the VDC program within the AAA.
• Serve as primary point of contact for Veterans, authorized representatives, caregivers, VA personnel, and partners.
• Coordinate referrals, intake, eligibility, enrollment, and ongoing participant services.
• Maintain procedures, workflows, forms, and documentation; monitor service delivery.
• Coordinate caregiver payments and ensure compliance with program requirements.
• Participate in meetings and required trainings; address concerns and action items.
