Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations within a neighborhood primary care network. Led by large care teams, providers focus on quality over volume to enhance patient experience and outcomes.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
• Internal Medicine or Family Medicine board certification (or eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain EMR data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
