Oak Street Health in Cleveland, OH is seeking a Primary Care Physician to serve Medicare populations at our neighborhood centers. You will join a large care team focused on value-based care, preventive medicine, and high-quality outcomes rather than volume.

Qualificações

  • MD or DO graduate
  • Board Certification in Internal Medicine or Family Medicine (or eligible)
  • Active, unrestricted State License
  • Active DEA license
  • US work authorization

Responsabilidades

  • Assess and diagnose patients at local clinics
  • Oversee, direct, and administer primary care
  • Prescribe and administer pharmaceutical treatments
  • Maintain Patient EMR data via Canopy and Greenway
  • Collaborate with leadership to meet health quality goals
  • Direct and manage the center care team with Practice Managers
  • Other duties as assigned
