Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based healthcare to local Medicare populations within our neighborhood primary care centers. You will diagnose and treat patients, work within a collaborative care team, and focus on outcomes that reduce hospital admissions and improve preventive care.

New graduates are encouraged to apply; experienced physicians may qualify for leadership roles.

Qualifications

• MD or DO physician licensed to practice in the US.
• Board certification in Internal Medicine or Family Medicine, or board-eligible.

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
