Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare patients. You will work within a neighborhood primary care center supported by a care team to improve quality and outcomes.

New graduates are encouraged to apply, with potential consideration for a Center Medical Director role. Anticipated hours are 40 per week in a full-time role, with comprehensive benefits and a strong focus on Patient care.

Qualifications

• MD or DO graduate with licensed board eligibility.
• Board Certification in Internal Medicine or Family Medicine (or eligible).
• Active, unrestricted State License and DEA license.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Perform other duties as assigned.
