Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations at our Chicagoland network. You will work with a large care team to provide high-quality care in a neighborhood clinic, focusing on outcomes and preventive medicine rather than volume.

You will diagnose and manage care, collaborate with leadership to hit quality goals, and participate in a comprehensive benefits program for full-time colleagues.

Qualifications

• MD or DO degree required.
• Board certification in Internal Medicine or Family Medicine or eligibility.
• Active, unrestricted state medical license.
• Active DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications and treatments.
• Maintain patient records in EMR systems.
• Collaborate with leadership to meet quality goals.
• Direct the care team with Practice Managers.
• Other duties as assigned.

Education
