Job summary

Oak Street Health in the Chicagoland area is seeking a Primary Care Physician to deliver value-based, patient-centered care to local Medicare populations within our neighborhood clinics.

You will work with multidisciplinary care teams to provide equitable, high-quality primary care, emphasize preventive medicine, and thrive in a transparent, outcome-driven bonus structure with comprehensive benefits.

Qualifications

• MD or DO degree and board certification or eligibility
• Active US medical license (unrestricted)
• Active DEA license
• US work authorization

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
