Job summary

Oak Street Health is seeking a Primary Care Physician to deliver value-based care in our neighborhood clinics serving local Medicare populations. You will work with large care teams to focus on quality outcomes rather than volume and participate in a comprehensive benefits package.

New graduates are encouraged to apply, with potential for Center Medical Director roles for highly qualified candidates. Anticipated weekly hours are 40 in full-time roles across Chicagoland sites.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate.

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
