Job summary

Oak Street Health in the Chicago area seeks a Primary Care Physician to deliver equitable, value-based care to local Medicare beneficiaries at our neighborhood centers. You will work within a supported team to focus on outcomes and preventive medicine.

Qualifications

• MD or DO degree and board certification in internal or family medicine (or eligible).
• Active, non-probationary, unrestricted state medical license.
• Active DEA license and US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee and administer primary care.
• Prescribe and manage medications.
• Maintain EMR data in canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.

Education

MD or DO graduate

Board certification in Internal Medicine or Family Medicine (or board eligible)

Active, unrestricted State License

Active DEA license

US work authorization

Geriatrics fellowship (preferred)

Geriatrics experience (preferred)

Value-based care experience (preferred)

Collaborative care experience (preferred)

Outpatient primary care experience (preferred)

Bilingual proficiency (where applicable)
