Qualifications

• MD or DO degree and medical license in good standing.
• Board certification in Internal Medicine or Family Medicine (or eligible).
• Active, unrestricted State license and DEA license.
• US work authorization required.
• Familiarity with value-based care is a plus.

Responsibilities

• Assess and diagnose patients at local Chicago clinics.
• Oversee and administer primary care with care teams.
• Prescribe and manage medications as appropriate.
• Maintain patient EMR data using Canopy/Greenway.
• Collaborate with leadership to meet quality goals.
• Direct center care team with Practice Managers.
• Additional duties as assigned.
