Oak Street Health in Burton, MI is seeking a Primary Care Physician to deliver equitable, value-based care to Medicare populations within our neighborhood centers. Qualifications

• MD/DO graduate required.
• Board certification in Internal Medicine or Family Medicine preferred or board-eligible.
• Active, unrestricted State License required.
• Active DEA license required.
• U.S. work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient EMR data via canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
