Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable value-based care to local Medicare populations across its neighborhood clinics. Providers work with large care teams to improve outcomes and patient experience in a center-focused model.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.).
• Graduate Internal Medicine or Family Medicine Board Certification (or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at Oak Street Health clinics.
• Oversee, direct, and administer primary care services.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
