Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations across its neighborhood clinics. You’ll join a collaborative team supported by center care leaders and large care teams, focusing on quality outcomes over volume.

Formación

• M.D. or D.O. required; board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsabilidades

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
