Oak Street Health is hiring a Primary Care Physician to provide equitable, value-based care to local Medicare populations at our neighborhood clinics. You will join a large care team focused on quality of care rather than volume, with outcomes-based bonuses to reward performance.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
• Internal Medicine or Family Medicine board certification (or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain patient medical records.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
