Oak Street Health is seeking a Primary Care Physician at our Philadelphia center to deliver equitable value-based care to Medicare patients. You will work with a care team at a neighborhood center to improve outcomes and reduce hospitalizations while focusing on preventive medicine.

Qualifications

• Must hold MD or DO and be board-certified (or eligible) in Internal or Family Medicine.
• Active, unrestricted State License and DEA registration.
• US work authorization and alignment with Oak Street Health values ('Oaky').

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments.
• Maintain EMR data in Canopy and Greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct center care teams.
• Perform other duties as assigned.
