Oak Street Health is seeking a Nurse Practitioner to provide effective, equity-focused primary care to adults on Medicare. You will work with a collaborative care team to deliver longitudinal care within outpatient settings, emphasizing strong patient relationships and evidence-based guidelines.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult Nurse Practitioner, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with holistic documentation.
• Coordinate care with physicians, specialists, and care facilities.
• Lead population health initiatives with the care team.
• Assist with triage, outreach, and patient education.
• Educate patients about conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
