Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, equitable value-based primary care to adults on Medicare. You’ll work on a collaborative care team, see patients independently when appropriate, and help patients stay healthy and out of hospital settings.

You will conduct visits, lead population health initiatives, and engage in care coordination with physicians, specialists, and care managers in outpatient clinics. Home visits may be part of the duties.

Qualifications

• MSN, DNP, or NP certificate graduate.
• Active state NP license (non-probationary).
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and HRAs.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the care team.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties, as assigned.
