Job summary

Oak Street Health is recruiting a Nurse Practitioner to deliver effective, equitable value-based primary care to adults on Medicare. You will work on a collaborative care team, providing longitudinal care and building strong patient relationships to keep patients healthy and out of the hospital.

You will conduct visits, coordinate care with physicians and specialists, and participate in population health initiatives while educating patients and families about conditions and treatments.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non‑probationary state Nurse Practitioner license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments, with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
