Job summary

Oak Street Health is seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable value-based primary care to Medicare patients in outpatient settings. The NP will work on a collaborative care team with physicians, NPs or PAs, medical assistants, and care managers.

The role emphasizes longitudinal care, patient education, and population health, with potential home visits and involvement in promotional activities.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
