Job summary

Oak Street Health is seeking a Nurse Practitioner to provide effective, equitable value-based primary care to adults on Medicare. You will work within outpatient clinics on a collaborative care team with physicians, NPs, PAs, MAs, and nurses to deliver longitudinal care and build trusted patient relationships.

You will conduct visits, coordinate care with specialists, schedule wellness visits, and participate in population health initiatives to improve outcomes and reduce hospitalizations.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, facilities, and agencies.
• Population health leadership in coordination with the Care Team.
