Oak Street Health is hiring a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare. You will see patients independently, collaborate with physicians, and contribute to population health in outpatient settings.

The role emphasizes comprehensive wellness, care coordination, and education, with a focus on building trusted patient relationships and supporting a team-based care model.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties as assigned.
