Job summary

Oak Street Health is hiring a Nurse Practitioner (Advanced Practice Provider) to deliver effective, equitable value-based primary care to adults on Medicare. You will work within a collaborative care team in outpatient settings, performing wellness visits, care coordination, and disease management to improve patient health outcomes.

Responsibilities include conducting visits, leading population health initiatives, triaging via phone, and educating patients.

Qualifications

• Requires MS/NP certification and active state NP license.
• DEA license and US work authorization required.
• Experience in primary care and population health is preferred.

Responsibilities

• Provide effective primary care within a value-based care model.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, testing facilities, and agencies.
• Lead population health initiatives in collaboration with the care team.
• Assist with triage and outreach by phone.
• Educate patients about health conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
